Organizations that have received funding as part of the Ocean County Cultural & Heritage Commission Regrant Program are obligated to provide credit in their promotional materials in exchange for the funding they receive.“(Organization Name) received funding this year from a grant administered by the Ocean County Cultural & Heritage Commission from funds granted by the New Jersey Historical Commission, a division of the Department of State.”The New Jersey Historical Commission (NJHC) and the Ocean County Cultural & Heritage Commission (OCCHC) logos must be used by the grantee in publicizing those programs which are supported by this grant. The logos must always be reproduced as units without alteration, never separating the marks from the type.

REQUIRED ATTACHMENTS:

● Organizational Long-Range Plan [Except Unit of government, institutions of higher learning or not primarily history based]: Long-range plans must include date of adoption by the Board of Directors.
● ADA Plan with Grievance Policy/Procedures [Required for all applicants who have applied for more than two years] All ADA plans must include date of adoption by the Board of Directors. The submitted ADA plans must address the issues covered in the ADA Facility & Program Accessibility Survey. Please contact C&H for assistance.
● Copy of your organization’s current IRS 990 Form [Required for non-profits only] An IRS 501(C) 3 Tax Determination Letter can be substituted by applicants in existence for less than 2 years. 


PART FIVE: SUPPORT MATERIALS

You may submit up to 5 items from the past 12 months. Material should be unbound and fit the 8 ½ x 11 format and support statements presented in the narrative.
The peer reviewers will receive these items electronically. Applicants may submit support materials on a CD/DVD/Flash Drive.​
