REQUIRED ATTACHMENTS:
● Organizational Long-Range Plan [Except Unit of government, institutions of higher learning or not primarily history based]: Long-range plans must include date of adoption by the Board of Directors.
● ADA Plan with Grievance Policy/Procedures [Required for all applicants who have applied for more than two years] All ADA plans must include date of adoption by the Board of Directors. The submitted ADA plans must address the issues covered in the ADA Facility & Program Accessibility Survey. Please contact C&H for assistance.
● Copy of your organization’s current IRS 990 Form [Required for non-profits only] An IRS 501(C) 3 Tax Determination Letter can be substituted by applicants in existence for less than 2 years.
PART FIVE: SUPPORT MATERIALS
You may submit up to 5 items from the past 12 months. Material should be unbound and fit the 8 ½ x 11 format and support statements presented in the narrative.
The peer reviewers will receive these items electronically. Applicants may submit support materials on a CD/DVD/Flash Drive.
Other support materials may include:
